Dear Families and Caregivers –

Below is information about a community survey currently gathering information about your experiences with developmental screening and service referral in King County. Your input is needed to inform a strategic plan for a King County developmental screening and referral process that will work for all families. Please consider taking 5-10 minutes to complete the survey before the end of this year.

The information you provide is confidential and cannot be linked back to you or your child. You will not be asked to provide any personal information.

Thank you, your time and responses are greatly appreciated!

Share your experience with developmental screening!

Developmental screening and referral is foundational to the well-being of young children, especially those birth through age 5. Cardea is partnering with WithinReach, King County Developmental Disabilities Division, and a coalition of community-serving organizations to learn about developmental screening and connections to services through the voter-approved Best Starts for Kids levy. We want to hear from you!  Participating in this 5-10 minute survey will help inform a universal developmental screening and referral system in King County to ensure all children and their families receive appropriate screening and linkage to supportive services, so more young people grow up to be happy, healthy, and thriving adults.

You can take the survey anytime between now and the end of December 2018. Some early respondents will be eligible for $5 if you choose to provide contact information.

The information you provide through this survey is confidential can never be linked back to you or your child. You will not be asked to provide any personal information like your name, phone number, or immigration status.
