Please add identifying information on the first page, and then fill in the details on which IEP services were lacking during the closure, and which services were offered but not accessible due to the student’s disabilities. Feel free to adjust the template as needed.

Why do this?

  1. To alert the Office of the Superintendent of Public Instruction to potential systemic problems with special education delivery while distance learning is in effect.2. To help stem feelings of isolation among parents and students. Emotionally, it can be hard to file a complaint. Coordinated filings can help parents see they are not alone, and that their concerns are often not isolated cases.

It is important that OSPI and local school districts see trends that need to be addressed, especially while planning for school re-opening.
